WebOur community hubs are based throughout North Lincolnshire. They make it easier for residents to access information, advice and support, and take part in activities to improve their health and wellbeing. The hub spaces are open to everyone. They are a central point in the community with support and activities for all – young and old. WebThis is a new digital approach to creating and maintaining education, health and care plans (EHCP). The hub provides a secure platform for families, young people and practitioners to make their contribution to the development of EHC plans. This page provides guidance for families, schools and other practitioners in the use of the EHC hub.
